Period Clots. Bad or Normal? Here Is What I Tell Every Patient Who Asks
Share
This is one of the questions I get most often. A patient notices clots in her period and immediately wonders, is this normal? Should I be worried? Is something wrong with me?
The honest answer is: it depends. Small clots occasionally are not something to panic about. But large, dark clots that appear every cycle are your body communicating something, and in TCM, we take that communication seriously.
Here is how I think about it, and what I tell patients when they bring this up in their consultation.
The quick guide
Not all clots are the same
The size, colour and frequency of clots all tell a different story. Here is a simple way to think about it.
Generally fine
Small and occasional clots
Smaller than a 20 cent coin, appearing only occasionally and not every cycle. The body is simply clearing the uterine lining. Not a major concern on its own.
Worth paying attention to
Large, dark and frequent clots
Larger than a 20 cent coin, dark in colour, appearing regularly every cycle. In TCM this signals blood stasis, blood is not flowing freely through the uterus.
Seek support
Heavy clotting with pain
Large clots accompanied by significant cramping, very heavy flow or pain that disrupts daily life. This combination suggests a more pronounced pattern that benefits from proper assessment.
The clot is not the problem. It is a signal. What matters is understanding why the blood is stagnating in the first place.
The TCM perspective
What period clots mean in TCM
In TCM, healthy menstrual blood should flow smoothly, be a bright to dark red colour, and leave the body without forming large clumps. When blood stagnates and forms clots, it tells us that something is obstructing the smooth flow of Qi and blood through the uterus.
There are several patterns that commonly drive this. Getting the right one is what determines the treatment direction.
Pattern 01
Cold in the uterus
Cold causes blood to contract and slow down, the same way cold thickens liquid. Over-consumption of cold drinks, raw food or too much aircon are common contributors. Clots tend to be darker and accompanied by cramping that improves with warmth.
Pattern 02
Liver Qi stagnation
When the Liver cannot move Qi smoothly, often due to stress, emotional tension or frustration, blood flow is disrupted too. Clots associated with this pattern often come with PMS, mood changes and breast tenderness before the period.
Pattern 03
Qi deficiency
When Qi is weak, it cannot push blood through the body effectively. Blood moves slowly and pools. This pattern often comes with fatigue, light-headedness and a feeling of being drained during the period.
Pattern 04
Blood stasis
Sometimes the stagnation itself becomes the primary pattern. Old blood that was not properly cleared in previous cycles accumulates and contributes to increasingly heavy clotting over time.
When to take it seriously
Signs your clots deserve a proper assessment
Most women dismiss clots as just part of their period. These are the signs that suggest it is worth looking closer.
Clots larger than a 20 cent coin appearing every cycle. This is not a one-off. Something is consistently preventing blood from flowing freely.
Dark, almost black coloured clots. In TCM, very dark blood indicates old, stagnant blood that has been sitting too long before being cleared.
Clots accompanied by significant cramping. Pain and clots together almost always point to blood stasis and obstruction in the uterus.
Clots getting worse over time. If the clotting has been increasing in size or frequency cycle by cycle, the underlying pattern is building rather than resolving on its own.
Clots alongside fertility challenges. Blood stasis in the uterus directly affects the environment for implantation. If you are trying to conceive, this is worth addressing before anything else.
What we do about it
How TCM treats period clots
Treatment depends entirely on which pattern is driving the clotting. Two women with the same symptom may need completely different approaches. This is why we assess before we prescribe anything.
The goal of treatment
Restore smooth Qi and blood flow so the body can clear itself properly each cycle
Rather than just reducing clot size, we work on correcting what is causing the blood to stagnate in the first place. When that changes, the periods become smoother, the clots reduce, and the pain that often comes with them eases too.
Herbal medication
Prescribed to your pattern
For cold patterns we warm and circulate. For Qi deficiency we tonify and move. For Liver Qi stagnation we soothe and regulate. No single formula works for everyone.
Acupuncture
Moves what is stuck
Acupuncture improves blood flow to the uterus, relieves pain associated with clotting and helps regulate the cycle so blood clears more smoothly each month.
Uterus care
The most comprehensive option
Combines acupuncture, Fu Yang warming, abdominal tuina and body adjustment to warm the uterus, clear stasis and restore circulation from multiple angles in one session.
Lifestyle support
What you do every day matters
Reducing cold and raw food, staying warm especially around the abdomen, managing stress and moving regularly all support treatment and prevent stasis from rebuilding.
Food as medicine
What to eat and what to reduce
Especially in the week before and during your period, what you eat directly affects how freely blood flows.
Supportive foods
Warm and cooked foods, soups, stews, congee
Ginger, warms the uterus and promotes circulation
Red dates and longan, nourish blood
Black fungus and black sesame, support blood flow
Rose tea, soothes Liver Qi
What worsens stasis
Cold drinks and iced food, especially around your period
Raw food in excess
Refined sugar and processed food
Dairy in excess
Period clots are common. They do not have to be your normal.
Most women have been told clots are just part of having a period. Some of them are. But regular, large, dark clots that come with pain are your body asking for attention, and in TCM, that kind of attention can make a significant difference to how your period feels every single month.
If you have been dismissing your clots as just how it is for you, it might be worth having a proper look at what is actually driving them.

Frequently asked questions
Common questions about period clots and TCM menstrual health in KL
Not always. Small and occasional clots are generally not a concern. But large, dark clots that appear regularly every cycle, especially with pain or heavy flow, are worth addressing. In TCM, they signal that blood is not flowing freely through the uterus, and that underlying pattern is worth identifying and treating.
A rough guideline is anything consistently larger than a 20 cent coin. If you are passing clots that size regularly every cycle, it is worth getting a proper assessment. The size, colour and frequency together tell the most complete picture, a TCM consultation looks at all three.
Yes. Blood stasis in the uterus affects the quality of the uterine environment, including the lining that an embryo needs to implant into. If you have significant clotting and are trying to conceive, addressing the stasis first gives conception a much better foundation.
Most patients start noticing changes within two to three menstrual cycles of consistent treatment. The clots tend to reduce in size first, followed by improvements in colour and pain levels. For longer-standing patterns it may take a little longer. We track progress each cycle and adjust the treatment accordingly.
