"I have tried everything. New cleanser, new toner, cut out dairy, drink more water. Still breaking out."

I hear this constantly. Women sitting across from me, tired and lost, because they have been through product after product and their skin is still not cooperating. Some even came in with skin that got worse from trying too many things at once.

Here is the thing. Adult acne is rarely a skin problem. It is an internal problem that shows up on your skin. And until you address what is happening inside, no amount of cleanser is going to fix it for good.

What I see in practice
Three patterns most often behind adult acne
Pattern 01

Weak Spleen and Stomach

In TCM, the Spleen and Stomach are responsible for transforming and transporting everything you eat. When they are weak, from irregular meals, too much cold food and drink, overthinking, or just general exhaustion, they cannot process things properly. What does not get processed accumulates. That accumulation turns into phlegm and dampness in the body, and dampness has to go somewhere. Very often it surfaces on the skin as congested, cystic, or persistent breakouts that never fully clear.

This is one of the most common patterns I see in KL. Busy schedules, skipping meals, eating quickly, cold drinks all day. The Spleen takes a beating without most people even realising it.
Pattern 02

Liver Qi stagnation from stress

When stress is high and not properly released, the Liver's ability to keep energy flowing smoothly gets disrupted. Stagnant Qi generates heat, and heat rises. It rises to the face. This tends to show up as red, inflamed breakouts. The angry kind that appear right before your period or during a particularly stressful week at work.

I see this constantly in working women in KL. The stress is real, the workload is real, and the skin is just the most visible place it shows up.
Pattern 03

Heaty constitution

Some people simply run hot. Too much spicy food, fried food, lack of sleep, or a naturally heat-prone constitution. All of this generates excess heat in the body that needs an outlet. The face is usually it. This is the pattern most people try to manage through diet alone, cutting out this and that. It helps at the edges but does not address the root.

When a patient comes in for acne, I spend a lot of time understanding the full picture. Where the breakouts appear, what they look like, when they flare, what makes them worse. Her digestion, her stress levels, her sleep, her cycle. Her tongue. Each answer fills in another piece of the picture.

Treatment usually involves a combination of acupuncture and herbal medicine, both working from the inside to correct the root imbalance rather than suppress the symptom.

The needles are extremely fine. Most patients are surprised by how relaxing it actually is. Some fall asleep during the session.

That is something I tell every patient who comes in for acne. Whatever is happening internally has usually been building for a while before it shows up on your face. And it takes time to clear properly from the inside out. But when it does clear, it stays clear. Because the root cause is gone, not just managed.
