The Fertility Conversation Nobody Wants to Have

"No la, must be my problem one. My husband is fine."

That is honestly one of the first things I hear when a couple comes in. The wife walks in alone, already convinced she is the reason things are not working. The husband stays home.

So the woman comes in, goes through everything, sometimes for months. Then I ask — has your husband done any checks? The answer is almost always no.

There are a few assumptions like this that I want to address before we get into the signs your body may have already been giving you.

Three things most people get wrong about fertility.

"Must be my problem one. My husband is fine."
Fertility is a shared equation. Male factors contribute to roughly half of all fertility challenges. Both partners need to be assessed. Coming in alone and assuming you are the problem is one of the most costly mistakes I see.

"My period comes every month, so my ovulation must be fine."
A regular cycle does not automatically mean healthy ovulation. You can bleed every 28 days and still have issues with egg quality or ovulation timing. The period arriving tells us one part of the story, not everything.

"I'm only in my late 20s. I have plenty of time."
Reproductive health is not something you switch on when you decide you are ready. It has been developing, or quietly declining, for years. The earlier you understand what is happening, the more options you have.

Painful periods

Pain severe enough to need painkillers, make you cancel plans, or keep you in bed is not normal. It is common, yes. But common and normal are not the same thing. In TCM, painful periods often point to patterns that affect how the uterus receives and nourishes an embryo.

What I commonly hear: "I just take panadol and push through." That pain is a signal. Signals are worth investigating.

Large blood clots

Most women do not even mention this until I ask specifically. Frequent large clots tell me that blood is not flowing as freely as it should. In TCM this is often associated with blood stasis, and blood stasis in the uterus is something we take seriously when discussing reproductive health.

Your period blood is giving you information every single month. Most women have never been taught how to read it.

Irregular cycles

If your cycle varies significantly, skips entirely, or is completely unpredictable, it is worth paying attention to. And remember, a cycle that looks regular does not always mean ovulation is happening at the right time.

Very light flow

Extremely scanty flow can point to blood deficiency in TCM, meaning the body may not be building a sufficiently nourishing uterine lining each cycle. For conception, the lining matters enormously. An embryo needs something to implant into.

PMS that significantly affects your daily life

Mood swings, breast tenderness, bloating, anxiety the week before your period. In TCM, significant PMS often points to Liver Qi stagnation, which affects the smooth flow of energy and blood through the reproductive system. I see this constantly in busy professionals in KL. Stress does not stay in your head. It lands in your body, and your cycle is usually the first place it shows up.
