Endometriosis in Malaysia: Why Painkillers Are Not Enough and What TCM Does Differently

"It is so painful, you know? I feel nauseous. And sometimes even numb on half my body."

That was the first thing she said when she sat down for her consultation.

She had been diagnosed with endometriosis. The doctor told her she could manage it with painkillers. But she did not want to take them. She was afraid of becoming dependent. And every time she brought up how bad the pain was, everyone around her said the same thing: it is normal.

It is not normal. It is common. But common and normal are not the same thing.

Most of the women who come to see us for endometriosis waited a long time before booking. They pushed through cycle after cycle, tried painkillers, came off them, and found themselves back where they started.

If that sounds like you, this post is for you.

Understanding the condition

What is endometriosis?

Endometriosis is a condition where tissue similar to the lining of the uterus grows outside of it. It can appear on the ovaries, pelvic lining, fallopian tubes, bladder or bowel. Each month, just like the uterine lining, this tissue thickens and breaks down. But because it is outside the uterus, it has nowhere to go. It cannot leave the body through menstruation, so it stays, causing inflammation, pain and bleeding in the pelvic area.

Symptoms you may be experiencing

Severe cramps and pelvic pain Heavy or abnormal bleeding Bloating Painful urination Painful bowel movements during period Nausea Numbness or radiating pain Fertility challenges

Conventional treatments include painkillers, NSAIDs, or hormone therapies that work by stopping menstruation. These can help for a period of time, but the effect is often not long lasting. And hormone therapies that stop menstruation are not suitable for women who are planning to conceive.

Most women end up back where they started the moment they stop the medication. The symptoms were managed. The root was not.
The TCM perspective

How TCM views endometriosis

In TCM, endometriosis is caused by an underlying imbalance that disrupts the smooth flow of Qi, blood and fluids. This disruption leads to blood stasis and phlegm-dampness that obstruct the flow in and out of the uterus, affecting how the endometrial lining grows and sheds each cycle.

When Qi and blood cannot flow smoothly, the pelvic area loses its warmth. That is when pain becomes more likely, and more severe.

The root causes of this stagnation vary from person to person. Here are the four most common patterns we see:

Cause 01
Cold in the uterus
From over-consumption of cold and raw food. Cold slows circulation and causes blood to stagnate.
Cause 02
Kidney Yang deficiency
The body lacks the warming energy to keep blood and fluids moving freely through the reproductive system.
Cause 03
Liver Qi stagnation
Often triggered by stress or emotional factors. When the Liver cannot move Qi freely, blood flow suffers too.
Cause 04
Spleen deficiency
A weak Spleen fails to process body fluids properly. Excess dampness accumulates in the pelvic area, leading to inflammation.
TCM does not make existing endometriosis disappear. But it can help limit its growth, relieve symptoms significantly, and restore the conditions your body needs to manage it better.
Treatment 01

Herbal medication, tailored to your cycle

One of the things that makes TCM treatment for endometriosis different is that herbal medication is adjusted across the four phases of your menstrual cycle. Your body has different needs at each stage, and your herbs should reflect that.

Pre-period · 5 to 7 days before
Prepare and regulate
Regulate menstrual flow to the uterus and pelvic area. Nourish Qi and blood. Soothe Liver Qi to prevent PMS and prepare for smooth shedding.
During period
Clear and relieve
Promote gentle uterine contractions to clear blood stasis. Regulate Qi and blood flow to relieve pain and reduce bloating. Clear dampness and inflammation.
Post-period · 5 to 7 days after
Warm and restore
Warm the uterus. Strengthen kidney and spleen. Support energy levels and recovery after the cycle.
Ovulation phase
Move and support
Ensure smooth transition of Yin to Yang. Promote Qi flow and clear blood stasis to support healthy ovulation and prevent recurrence of stasis.

Formulas are adjusted over time based on how your symptoms respond and how your cycle changes. Nothing is fixed, the treatment moves with your body.

Treatment 02

Acupuncture

During acupuncture treatment, fine sterile needles are placed at specific points to improve blood flow to the uterus and regulate Qi that runs through the Spleen, Liver, Kidney and Ren meridians, the meridians that govern reproductive health and menstruation.

Relieves pain by elevating endorphins, dopamine and serotonin, your body's natural pain and mood-relieving hormones
Limits abnormal tissue growth by facilitating the body's natural response to the endometrial tissue
Relieves gastrointestinal symptoms including bloating and constipation that often accompany the period
Reduces inflammation and eases pain throughout the pelvic region
Supports hormonal balance and helps regulate the menstrual cycle

What you can do at home

How to support your recovery

Treatment works best when it is supported by what you do every day. Three things make the biggest difference.

Food as medicine
What to eat and what to reduce
Eat more of
Warm and cooked foods, soups, stews, steamed vegetables
Ginger, red dates, longan
Warming herbal teas
Reduce
Cold and raw food, they worsen blood stasis
Refined sugar and oily food
Dairy products
Cold drinks, especially during and before your period

Exercise regularly, even gentle movement helps promote circulation. We have noticed that patients who exercise consistently see better improvement overall. And if you are between sessions, massaging these three acupoints for 3 to 5 minutes in a circular motion, 2 to 3 times a day, can help with pain relief and circulation.
